Enrollment

Before your 3-5 year old child can be enrolled with the preschool, we will need a completed enrollment packet (forms attached below) and the following documents:

Birth Certificate (original, no copy)

Immunization Records

Proof of Residence:

Rental/Lease agreement
Purchase/escrow agreement
Copy of utility bill

Notarized statement from owner/renter indicating:

a. Names of people living with owner/renter
b. Anticipated length of time of residence with owner/renter
